PLEASE NOTE:

Payment in advance is required for official transcripts. The cost is $5 for the first and $1 for each additional ordered at the same time. The transcript request form has an area at the bottom for you to provide your credit or debit card information.

We must have your signature in order to release a transcript for any reason, even to you. Education records are protected by the Family Education Rights and Privacy Act (FERPA).

We process requests in the order they are received. Certain times during the term have more requests than others.

If you are unable to download this form, you can submit a request, again either by mail or fax by sending us the following information.

• Social Security Number
• Name
• Address
• How many transcripts you need
• Where you want your transcripts sent
• Credit/Debit card information for payment
• Your signature is required!

There is a $5 fee for one official transcript and $1 for each additional one ordered at the same time. There is no fee for unofficial transcripts.
